Resumen : RESUMEN: Para evaluar la severidad clínica de la hemofilia y señalar su clasificación, se estudiaron pacientes con hemofilia
A y hemofilia B que acudieron a la consulta de hematología del Servicio Autónomo Hospital Universitario “Antonio
Patricio de Alcalá”, de la ciudad de Cumaná, estado Sucre, durante los meses de junio-julio de 2003. Fueron
procesadas 26 muestras de plasma sanguíneo de pacientes de sexo masculino con edades comprendidas entre 1 a
40 años. Se utilizó el método de cuantificación de los factores VIII y IX (en una etapa) para la determinación de
la actividad coagulante del factor deficiente en cada muestra. El 84,6% (22 casos) correspondió a hemofilia A, y el
15,4% (4 casos) a la hemofilia B en el grupo estudiado. Con respecto a la clasificación clínica, de acuerdo al grado
de severidad en pacientes con hemofilia A, la más frecuente fue la hemofilia severa representando el 50% (11
casos), seguida de la moderada con un 27,3% (6 casos) y la leve con 22,7% (5 casos). En la hemofilia B predominó
la hemofilia severa con un 75% (3 casos). La manifestación clínica de mayor frecuencia en este estudio, fueron
las hemartrosis en rodillas, tobillos y codos (77%), siendo la afectación de mayor porcentaje en los hemofílicos
severos. ABSTRACT: Twenty-six hemophilia patients of the hematology service of the Antonio Patricio de Alcalá University
Hospital, in the city of Cumaná, state of Sucre, were evaluated during the period June-July 2003 in order to
ascertain the type and clinical severity of their illness. All patients were male, their age ranging between 1 and
40. A one-stage quantification assay was used to determine the blood clotting activity of factor VIII and factor
IX deficiencies in each sample. Twenty-two cases, or 84.6%, resulted in type A hemophilia, and four cases, or
15.4%, in hemophilia B. As for the clinical classification, severe hemophilia was predominant among both groups,
namely, 11 cases, or 50%, among the former, and three cases, or 75%, among the latter. Six moderate cases, or
27.3%, and five slight cases, or 22.7%, completed the hemophilia A group. Hemarthrosis in knees, ankles, and

Resumen : RESUMEN: Las muertes violentas por suicidio e intento de suicidio según el Informe de las Tendencias de salud de
la Organización Panamericana de la salud, están adquiriendo una importancia creciente en todos los países,
convirtiéndose en un grave problema de salud pública. Con la finalidad de determinar la asociación causal entre
suicidio e intento de suicidio con los factores de riesgo, desintegración social, nivel Socioeconómico y enfermedades
previas físicas y mentales, y el porque de las lesiones auto-infligidas desde la perspectiva de los actores; se
utilizó la triangulación metodológica cuantitativa y cualitativa así como la caracterización epidemiológica. En el
estudio de casos y controles para un nivel de significancia de 0,05, una precisión relativa de 50% y un límite de
confianza de 95% la muestra quedó conformada por 114 individuos. Para el estudio cualitativo se trabajó con una
muestra opinática. La asociación entre suicidio y el factor de riesgo nivel socioeconómico, fue estadísticamente
significativa. La Odd Ratio evidenció una probabilidad cuatro veces mayor del suicidio en la población de los
niveles socioeconómicos IV y V. El riesgo de suicidio en familias desintegradas es 10 veces mayor que en las
familias estables. De igual manera, las personas con enfermedades físicas y trastornos de personalidad tienen
dos veces más riesgo de suicidarse o atentar contra su vida. Se concluyó que la falta de apoyo social, la familia
inestructurada, el bajo nivel socioeconómico y la carencia de afecto y apoyo familiar son determinantes en el
aumento del suicidio e intentos de suicidio.
ABSTRACT: According to a report on health tendencies by the Pan American Health Organization, completed suicides
and suicide attempts are increasingly becoming a serious public health issue in all countries. A study based
on the epidemiological characterization and methodological triangulation involving combined qualitative and
quantitative approaches was undertaken to ascertain the causal relationship linking successful and unsuccessful
suicide attempts with social disintegration, impoverished socioeconomic status, and a history of physical and
psychiatric illnesses. The study attempts to view self-destruction from the perspective of the perpetrator. A case-
control study involving 114 individuals yielded the standard level of significance of 0.05, a relative precision of
50%, and a 95% confidence interval. An opinion poll provided the basis for the qualitative component of the study.
The odds ratio reflected a five-fold likelihood of suicide among the members of socioeconomic groups IV and V,
and a risk for suicide in broken families 11 times as likely as that of stable ones. Likewise, subjects with physical
and personality disorders have three times as much chance of committing suicide or attempting against their lives
as that of carefree people. The lack of social backing, a broken home, a deprived socioeconomic environment, and

Resumen : RESUMEN: A fin de evaluar la relación entre el derrame de bilis- cálculos al momento de realizar la colecistectomía
laparoscópica y la aparición de complicaciones infecciosas; en estudio descriptivo prospectivo comparativo de 558
pacientes en quienes se realizó la colecistectomía laparoscópica por enfermedad biliar, en el servicio de Cirugía
General número 1 Hospital Universitario Miquel Pérez Carreño Caracas Venezuela, desde Octubre de 1991
hasta Noviembre del 2004. Los criterios de exclusión fueron: colecistectomía abierta; conversión de la cirugía
laparoscópica a cirugía tradicional. Se tomaron muestras de bilis y de fragmentos de la pared vesicular incluyendo
la mucosa; para cultivos de gérmenes aerobios y anaerobios. A los pacientes con enfermedad vesicular avanzada
(n = 160- 28,69%) se les administró en el preoperatorio terapéutica con (Cefalosporina más metronidazol). A los
de alto riesgo con enfermedades asociadas, ceftriaxone 1 gr antes de la inducción anestesia (n = 42). Sé determinó
la frecuencia de los cultivos positivos, y las complicaciones infecciosas. 52 cultivos de la bilis (9,31%), y 102
(18,27%) de las muestras de la pared vesicular resultaron infectadas (p= 0,05). En 4 pacientes se presentaron
infecciones menores, en el portal subumbilical (0,71%). El porcentaje total de complicaciones infecciosas fue
insignificante y no se correlacionó con la presencia de bacterias en la bilis o en el cultivo de fragmentos de
la pared vesicular. Resultado que se atribuye a la terapia antibiótica agresiva indicada especialmente en los
pacientes de alto riesgo. ABSTRACT: To assess the incidence of infected bile and gallbladder wall infection at the time of laparoscopic
cholecystectomy, and find out if they influenced the rate of postoperative infective complications. A prospective
descriptive comparative study the all 558 patients who had their gallbladders removed laparoscopically for
gallbladder disease in the university hospital Miguel Pérez Carreño Caracas Venezuela between October 1991
to November 2004. Samples of bile and gallbladders wall were taken from all patients and cultured separately
for aerobic and anaerobic bacteria. Patients with complicated gallbladder disease (n =160-28,69%) were given
preoperative therapeutic antibiotics ( cephalosporin plus metronidazol); and other high-risk patient 42 were given
prophylactic ceftriaxone either 1 g x 3, or a single dose. Main out come measures were, incidence of infected
cultures, and infective morbidity. 52 specimens of bile ( 9,31%) and 102 ( 18,27%) specimens of gallbladder wall
were infected. Four patients in whom neither specimen had shown any growth develop minor infections at the
umbilical port. No patient in whom either specimen was infected develop an infective complication. The overall
rate of infective complications not correlate with the presence of bacteria in the bile or gallbladder wall. This is

Resumen : RESUMEN: Se identifica la zona de cambio de régimen sísmico, de subductivo (profunda), a rumbo deslizante
(superficial) en la esquina sur oriental de la placa del Caribe. Se utilizaron 1226 sismos registrados por la Red
Sismológica Nororiental de Venezuela (ReSudo), administrada por el Centro de Sismología de la Universidad
de Oriente (CSUDO), en el periodo de 1995 a 2001, ubicados entre las coordenadas 9 y 12 N, y 61 y 64 W.
Se dividió esta región en cinco sectores paralelos y tres perpendiculares a la dirección noroeste sureste. Las
secciones rectangulares paralelas son aproximadamente de 200 por 60 kilómetros cuadrados, mientras que los
perpendiculares son de poco más o menos 350 por 60 kilómetros cuadrados. Obteniéndose por separado los
perfiles de sismicidad en ambas direcciones, para así detectar dónde la profundidad sísmica cambia. Se observa
en los segmentos paralelos a la dirección NO-SE una sismicidad profunda centrada en 10,2° N y –62,8 W que
por su distribución y profundidad parece corresponder a la zona oriental del Corrimiento de Pedernales con un
buzamiento promedio de 40° al noroeste. Las profundidades se incrementan hacia el Noreste del sistema de fallas
de El Soldado – Los Bajos, identificados como el límite entre el régimen superficial (rumbo deslizante) y el más
profundo (subductivo), teniendo este grupo de fallas una actividad delimitada hasta los cuarenta kilómetros de
profundidad. El último de los segmentos paralelos presenta un evento de magnitud 6,5 y profundidad de
40 kilómetros, característico de una zona de subducción, que parece buzar al noreste con un acimut de 350o.
Se usaron dos métodos gráficos para evidenciar la frontera entre los régimenes, en primer lugar un representación
en contornos de la profundidad media (análisis espacial) y en segundo lugar, un corte promedio por perfil. ABSTRAC: Using the data base of the Venezuelan Northeastern Seismological Network (ReSUDO for its Spanish
acronym), which is under the administration of the Seismology Center of the Universidad de Oriente (CSUDO),
we were able to identify the zone of seismic regime change, from subduction, or deep seismicity, to dextral, or
shallow seismicity in the southeastern corner of the Caribbean plate, an area located between latitudes 9 and 12
N and longitudes 61 and 64 W. The study was carried out from 1995 to 2001 with the data from 1226 seismic
tremors registered by the aforementioned network. The area was divided in five parallel and three perpendicular
zones to the northwest-southeast direction, the former measuring about 200 by 60 square kilometers and the latter
about 350 by 60 square kilometers. Separate seismic profiling in both directions enabled us to identify where
the seismicity changes from shallow to deep. A deep seismicity centered at 10.2oN and 62.8 W is observed in
the parallel segments, its distribution and depth seemingly corresponding to the eastern zone of the Pedernales
Thrust system with a northwestward mean deep of 40o. The depths of the foci increase northeastward from the
El Soldado-Los Bajos fault system, which borders on both seismic regimes and whose activity reaches a depth of
40 km. Typical of a subduction area, seemingly thrusting in a northeasterly direction with an azimuth of 350o, the
last of the parallel segments shows an event with a 6.5 magnitude at a depth of 40 km. Two graphic methods were
used to map the boundary between the regimes, namely, a contour representation of mean depth (spatial analysis),
and an average activity profiling breakdown.2005-01-01T00:00:00Z
